How to create your ATF eForms account:(Required)

1. Go to https://eforms.atf.gov/login

2. Scroll to the very bottom of the page and click the green “Register” button.

3. Follow the prompts and be sure to fill out all information.

4. When complete, at the bottom of the page click register.

5. You should receive an email from eForms confirming your registration.

6. Please notate your username and PIN number and bring that with you. *username should be first initial, first 5 letters of your last name, followed by 5 numbers*

Please remember your username and pin that you created on the ATF eform website. You will need it to submit your forms. **Already have an ATF eForms account? I acknowledge I need to bring a credit card with me to purchase my $200 tax stamp, paid to the ATF.

Acknowledgement of Photo & Fingerprinting:(Required)

I acknowledge that I am required to have a photo taken of me to upload onto my Form 4 application. I also acknowledge that I will be fingerprinted to also submit my Form 4 application.

A head-shot photo will need to be taken in order to upload onto your electronic Form 4. Two fingerprint cards will be utilized to submit to the ATF via mail.
